What Is Hallux Rigidus?

Hallux rigidus occurs when the cartilage in the big toe joint gradually wears down over time. As the joint becomes arthritic, motion decreases and bone spurs may develop, further limiting movement.

Unlike bunions, which are a deformity of alignment, hallux rigidus is primarily a degenerative joint condition that worsens over time if not properly managed.

What Are The Common Symptoms?

  • Pain at the top of the big toe joint

  • Stiffness and limited upward motion

  • Swelling around the joint

  • Pain with walking, especially during push-off

  • Difficulty wearing certain shoes

  • Symptoms often progress gradually and may worsen with activity.

What Are The Causes And Risk Factors?

Hallux rigidus can develop due to:

  • Prior injury to the big toe joint

  • Repetitive stress over time

  • Abnormal foot mechanics

  • Genetic predisposition

  • Inflammatory arthritis

As joint damage progresses, motion becomes increasingly limited.

What Are The Treatment Options?

Most patients improve with conservative treatment.

Conservative Treatment​

  • Shoe modifications (stiff or rocker-bottom soles)

  • Carbon fiber inserts to limit joint motion

  • Anti-inflammatory medications

  • Activity modification

  • Corticosteroid injections in select cases

  • These treatments help reduce pain but do not reverse arthritis.

Surgical Treatment

  • Surgical treatment depends on the severity of arthritis and patient goals.

  • Common procedures include:

  • Cheilectomy (removal of bone spurs) for early-stage disease

  • Joint preservation procedures in moderate cases

  • Fusion (arthrodesis) for advanced arthritis

  • There is no single procedure appropriate for every patient. Surgical planning is individualized based on pain level, joint damage, and activity demands.

What Is The Recovery Timeline?

Many patients improve within several weeks once proper treatment begins.

Surgical recovery depends on the procedure performed.

  • Cheilectomy patients often return to walking relatively quickly

  • Fusion procedures require a longer recovery but provide reliable pain relief
